Who lives in your household? (Adults, children, ages) Does anyone have cat allergies? Do you own or rent your home?

If renting, we may ask for confirmation that pets are allowed.

Do you have a garden/outdoor space? If yes, is it safe/secure?

Section 3: Your experience with pets

Have you owned cats before? If yes, tell us a little about them. Do you currently have any other pets? (Species, ages, temperament) How do you think your current pets might react to a new cat?

Section 4: Lifestyle & care

How long are you typically away from home each day? Where would the cat stay when you’re out or at night? Are you looking for an indoor-only or indoor/outdoor cat? Are you open to adopting a cat with special needs? If yes/maybe, tell us anything you’re comfortable with (e.g., medication routines, mobility aids)

Section 5: Matching & preferences

What kind of cat are you hoping to adopt? (Age, temperament, personality) Are you open to siblings/pairs? Would you consider a senior cat or one with extra needs?

Section 6: Commitment

Are you prepared for the costs of cat ownership? What arrangements would you make if you go on holiday? Have you ever had to rehome a pet before? (If yes, please explain)

Section 7: Agreement

Do you agree to a home visit (in person or virtual)? Do you agree not to sell or pass on the cat but return them to Whiskers Health if needed? Do you understand that completing this form does not guarantee adoption?

By applying, you confirm you’re prepared for the responsibilities and costs of cat ownership. If things don’t work out, you agree to return the cat to Whiskers Health Organisation. We may arrange a home visit before adoption.
